The CMH Urology Clinic provides a full range of procedures and treatments for a variety of urological conditions. Our urology specialists work closely with patients and their primary care provider to develop the best treatment plan, taking into consideration other conditions that may be treated simultaneously.

Q: How do I get my prescription refilled? click to open
A: If you need a refill of your medication, please call your pharmacy and ask them to submit a refill request. Please allow three (3) business days for all refill requests, and see your provider on schedule so that we can monitor your health and keep your prescriptions up to date and refilled as needed.
Q: Are your services covered by insurance? click to open
A: Some services may not be covered by your insurance plan. Please ask your medical provider and your insurance provider about costs before planning any procedure. Please note that you may designate a Care Partner to assist you with managing your healthcare at CMH.
