Striving to be your healthcare provider for life

Primary Care Clinic

Good Health Begins with Prevention

It is important to have a primary care provider, whether that person is a doctor, nurse practitioner or physician assistant. A primary care provider is the leader of your healthcare team. Their knowledge of your health over time is invaluable.

CMH Primary Care Clinic’s doctors and nurse practitioners focus on developing long-term partnerships with their patients and promoting a lifetime of good health. They are trained in all major medical areas, allowing them to address a variety of medical conditions, and practice evidence-based care for effective outcomes. When necessary to diagnose or treat a condition, they can refer to specialists.

CMH Primary Care Clinic is recognized by the Oregon Health Authority as a patient-centered primary care home, in keeping with the patient-centered approach of all Columbia Memorial Hospital (CMH) clinics.
