Nominate a Nurse for the DAISY Award

Want to say ‘Thank You’ to your nurse?

You can show your gratitude by nominating a compassionate and skilled nurse for The DAISY Award For Extraordinary Nurses (The DAISY Award).

The DAISY Award is an international program that rewards and celebrates extraordinary clinical skill and compassionate care provided by nurses. Created in 1999, more than 2,400 healthcare facilities in all 50 states, as well as 14 other countries, use the DAISY Award to honor nurses.

Has a nurse at Columbia Memorial Hospital or one of the CMH clinics gone above and beyond in your care? Did they make you feel comfortable while at the hospital, or go the extra mile to make your smile? Recognize them here! Tell us your story of care!

Tell us how your nurse made a difference in your experience

We will let you know if your Nominee is selected. (All Nominees are celebrated, so please tell us how your Nurse made a special difference in your visit with us.)
