Who will assist in my care at CMH?

Caregivers from all over the hospital care for you during your time in the Swing Bed Care Program.

Physicians: Your primary physician may provide care for you, if he/she is credentialed to provide inpatient care at CMH and chooses to do so. Otherwise, our hospitalist will follow your care and visit you on an as-needed basis, no less than weekly, to assure your medical needs are met and to help plan your treatment.

Nurses: Our nursing team serves you 24 hours a day, seven days a week. The team helps you with activities of daily living, administering medications and treatments and helping you to recuperate.

Care Manager: A care manager works on your behalf to interface with external payor sources and review agencies and communicates with you before and after each meeting to ensure your involvement and prepare for discharge.

Rehabilitation: A physical therapist assesses your movement abilities and designs a treatment program to restore and maximize movement and function. An occupational therapist helps you learn the skills/changes needed for you to be productive, independent and satisfied at home and will meet with you to offer activities to participate in. A speech language pathologist works to improve your communication skills and address swallowing disorders.

Social Worker: A social worker will meet with you to identify ways to support you during your stay.
