Win the funds to purchase an Automated External Defibrillator (AED) during Heart Month!

If someone went into cardiac arrest at your organization, would you be prepared? During Heart Month, February 2021, CMH is giving away the funds to purchase two Automated External Defibrillators (AEDs) to local organizations who can tell us why they should have an AED on site. #communityfirstaid

Basic Information

February is recognized as American Heart Month. During February 2021, enter to win $1,300 toward the purchase of AED equipment for your organization.

More than 356,000 out-of-hospital cardiac arrests (OHCAs) happen in the United States every year. In a 2018 study, use of AEDs by bystanders of OHCAs was associated with better survival and functional outcomes. CMH wants to help with this — with more AEDs and training available, there may be further improved outcomes of OHCAs.

Enter to win by filling out this survey and telling us (in 250 words or less) why having an AED on site at your organization would be helpful. Entries will be blind and winners will be chosen by the leadership of the WomenHeart of the North Oregon Coast heart disease support group in March 2021 and announced afterward.  


  • Any business/church/organization with a physical location in Clatsop County (OR), Tillamook County (OR), Columbia County (OR), Wahkiakum County (WA) or Pacific County (WA) is eligible to enter Columbia Memorial Hospital’s contest to win an AED.

  • Each entrant must write an essay of up to 250 words on why having an AED on site at their organization would be helpful. Only one entry per organization will be accepted. 
  • CMH WomenHeart group leadership will determine the winner of the contest at their March 2021 meeting. Winners will be contacted immediately following. 
  • Should either of the two winners decline to accept the funds to purchase an AED, the CMH WomenHeart group leadership will choose an alternate winner. 
  • Winners agree to have their photo taken and posted to any of Columbia Memorial Hospital’s social media pages and website online. They also agree to allow CMH to use the photos and their name and organization name in future marketing about AED giveaways.

Terms and Conditions

  • The AED promotion is sponsored by Columbia Memorial Hospital (“CMH”), 2111 Exchange St., Astoria, OR 97103.
  • 2 (Two) winners will be selected to receive $1,300 toward the purchase of AED equipment for their organization.
  • To enter the promotion, the entrant must complete the survey entry form found here and submit that form electronically no later than 5:00 p.m. P.S.T. on 02/28/2021 (“Deadline”). No other method of entry will be accepted. CMH’s computer is the official time keeping device. CMH is not responsible for entries that are corrupted or otherwise not received correctly or for any other problems or technical malfunctions that result in the failure of an entry to be correctly and promptly transmitted to CMH. CMH is not responsible for lost, late or misdirected entries. Entries received after the Deadline will not be accepted. 
  • Each organization can submit one entry form. 
  • The winner will be selected by a selection committee consisting of WomenHeart of the North Oregon Coast group leaders. The winners will be selected based on the essay submitted by each entrant, and any other factors deemed to be relevant to the committee (names of person entering and organization will be blind). Determination of the winners shall be made by WomenHeart and CMH and/or its agents in their sole and absolute discretion, and such determination shall be final and binding. Odds of winning depend on the actual number of entries received prior to the Deadline. 
  • No organization shall win more than once.
  • Winners will be notified by CMH by 04/30/2021 via telephone, email and/or U.S. mail using the contact information provided to CMH at the time of entry (“Winner Notification”). CMH will have no liability for any Winner Notification that is lost, intercepted, or not received by the winner for any reason. Each winner must be a legal U.S. resident 18 years of age or older at the time the winning entry was submitted, and must provide a valid driver’s license or official state identification in order to claim the AED.
  • The funds will be provided directly to the winning organizations for the sole use of purchasing AED equipment. The funds may not be used for any other reason and proof of purchase must be provided to CMH within six months of winning. If proof of purchase is not provided within six months, the funds must be returned to CMH. All federal, state and local taxes on the prize are the sole responsibility of the winner. CMH shall have no liability or responsibility with respect to fulfillment, shipment, delivery, or condition of the AED equipment.
  • Each winner must sign a Liability Waiver releasing CMH of all liability associated with the AED Promotion by the date listed in the Winner Notification in order to claim the funds to purchase AED equipment. 
  • Information provided by entrants as part of the promotion becomes the property of CMH and may be used by CMH in conformity with the CMH Privacy Policy. Each winner must sign a release allowing their photograph or likeness to be used on CMH social media pages and and related promotional materials (“Publicity Release”) in order to claim the funds to purchase AED equipment. 
  • Employees of CMH, their immediate family members (spouses and siblings, children and parents and their spouses, regardless of where they live), and members of their households (whether related or not) are not eligible to participate. 
  • CMH reserves the right to disqualify any entry. Disqualification of a winner may result from any of the following: (i) the winner’s refusal to accept the funds to purchase AED equipment; (ii) the winner’s failure to execute and return the Liability Waiver or Publicity Release by the deadline in the Winner Notification; (iii) the return of a Winner Notification as undeliverable; or (iv) any other non-compliance with these Rules. 
  • A list of winners will be posted on by 04/30/2021.
  • No purchase or subscription is necessary to win. Purchase of or subscription to any CMH-related publications does not increase chances of winning. 
  • CMH reserves the right to cancel, terminate, or suspend the AED promotion in the event that the promotion is unable to run as planned for any reason, including but not limited to infection of CMH’s computer(s) by computer virus, tampering, unauthorized intervention, fraud, and any other causes which corrupt or affect the administration, security, fairness, integrity, or proper conduct of the promotion.
  • By participating in the promotion, entrants agree to abide by these Rules and to release, discharge, and hold CMH and its employees and agents harmless from any and all claims, liabilities, losses and damages of any kind resulting from their participation in the promotion or their acceptance, use or misuse of the AED including, without limitation, personal injury, death and property damage. 
  • The promotion shall be governed exclusively by the laws of the State of Oregon. Void where prohibited or restricted by law.
